Most colleges and other educational institutes consider student feedback quite important for improving the quality of education they provide. While some colleges take feedback once a year or every semester, there are institutes that do this every month. Such feedback forms are usually in the form of questionnaires.Instead of writing the answer, students are provided with answer options which they can mark, making the survey easy to attend.
As a college has thousands of students, reading all the forms manually can be tiresome. Hence, most colleges prefer using OMR forms for this purpose. These forms are read by the OMR software, which transfers the data into Excel or any other output of your choice. Once this is done, you can use the data as you want.
Here, we have provided a questionnaire sample for survey of student feedback in college. This feedback is for the faculty of the college and is taken monthly.
At the top right of the page are the instructions for filling the sheet.
The students have the choice to mark from one to five for every question. Here, one stands for strongly disagree, two stands for disagree, three stands for not sure, four stands for agree and five stands for strongly agree.
This is followed by instructions for filling the bubble. Correct and incorrect methods of filling the bubbles are illustrated.
The students have to fill the complete bubble and not just tick mark or put a cross or any other sign on the bubbles. The students also have to provide their student ID in both written and bubble format.
Before answering the questionnaire, the students have to mention their program and specialization. As this is a monthly feedback, they also have to mention the month.
The name of each faculty member and the subject he/she teaches can be mentioned at the top of every column.
