Several big corporate houses conduct surveys on a regular basis to know the experience of the employees working there. As it is not possible for the heads of such big companies to interact with employees at all levels, this is the best way of knowing their feedback about their work and the company. To ensure that the employees provide their feedback openly, they do not need to write their name or reveal their identity although there is a pre-printed barcode at the end of the OMR sheet.
We have provided a sample of an OMR form used by Sahara, one of India's big corporate houses, to know the comfort level of their employees. The speciality of this form is that it is multi-lingual, i.e. it has two languages: English and Hindi. This is to ensure that even employees of the worker class would be able to participate in the survey.
The survey form consists of 15 questions and each question has four options. The survey form asks questions about the employee's relationship with his/her superior, whether their skills, knowledge and work are appreciated, whether they are able to maintain satisfactory work-life balance, etc.
The four parameters with which one has to provide feedback include fully agree, partially agree, partially disagree and fully disagree. The employees have to put a tick mark on the box they want to select for each question.
