Medical research companies conduct regular surveys to know about the growth of a disease, the effect of some medicines, etc. This research helps them to understand how a disease has affected a particular section of the population, whether there is need to introduce a new type of a medicine or to stop the supply of some drugs and such other things. Nowadays, most of these surveys are objective type and hence make use of OMR sheets.
The OMR sheets are special type of forms which can be read by a form reader. This rules out the need for manual checking or reading of these forms. They are first scanned and then read by the OMR software. This is a convenient and time saving method of processing the survey forms.
Here we have provided a sample of survey form of patients using OMR reader to check the growth of asthma as disease.
The survey form does not ask for any personal information from the patients participating in the survey.
There are in total 11 questions in the questionnaire and each one of them has sub sections or sub questions.
The parameters of answering each question are different. It depends on the nature of the question.
Patients are asked to share information about their asthmatic condition, the type of treatment they use and whether they would be interested in taking part in a similar survey in the future.
Instruct the patients to fill the bubbles completely; otherwise the OMR reader won’t be able to identify the markings.
